you are able to participate in this study if you:

i) are aged at least 18 years;

ii) have pain in your big toe joint for at least 3 months;

iii) have big toe joint pain rated at least 30mm on a 100mm pain scale;

iv) have pain on the top of your big toe joint;

v) are able to walk household distances (>50 meters) without the aid of a walker, crutches or cane;

vi) are willing to attend the Health Sciences Clinic at La Trobe University (Melbourne, Victoria) for an assessment on four occasions;

vii) are willing to attempt to not receive additional treatments (such as physical therapy, foot orthoses, shoe modifications, intra-articular injections, or surgery) for your big toe joint pain during the course of the study (12 months);

viii) are willing to attempt to discontinue taking all pain-relieving medications to relieve pain at your big toe joint (except paracetamol) for at least 14 days prior to the baseline assessment and during the study period (12 months);

(ix) are willing to discontinue use of any foot orthotics (if currently using).


YOU ARE NOT ABLE TO PARTICIPATE IN THIS STUDY IF YOU:
i) have had previous surgery on your big toe joint;
ii) are pregnant;
iii) have significant deformity of your big toe joint such as bunions;
iv) have any other painful conditions within the foot or ankle;
v) currently have inflammatory arthritis (including rheumatoid arthritis and gout) or any other connective tissue disease;
vi) have any medical condition that, in the opinion of the investigators, makes you unsuitable for inclusion;

vii) are unable to speak and read English;
viii) have cognitive impairment;

ix) have received intra-articular injections (such as corticosteroids) at the first MTPJ in the previous 3 months;

x) are currently wearing shoes that would not be able to accommodate the shoe inserts;

xi) are unable to reach your feet.

xii) have any conditions that do not allow the use of MRI. These conditions include:

cardiac pacemaker / pacing wires / defibrillator, tattoo/ body piercing / permanent makeup, artificial heart valve, vascular implant (eg stent/ embolisation coils), implanted drug infusion device, neurostimulator / biostimulator, anuerysm clips, intra-ventricular / intra-cranial / spinal shunt, Cochlear implant/hearing aid/ Stapes (ear) implant, tissue expander (eg breast), metallic implants/joint replacements/rods or screws, any metallic foreign bodies (eg shrapnel), intra-uterine device, any eye injury caused by metal, history of diabetes, kidney disease, kidney failure or are currently on dialysis, possibility of being pregnant, breast feeding).


INVESTIGATORS WILL ENQUIRE ABOUT THE CRITERIA AT THE INITIAL APPOINTMENT
